Cullinan Therapeutics Inc. (Nasdaq: CGEM) has dosed the first patient in a potentially registrational Phase 2 study of CLN-049 for relapsed or refractory acute myeloid leukemia.

The October 8 milestone moves the investigational T-cell engager into a study intended to determine whether its results could support a future regulatory application. The trial is enrolling patients who have received no more than two previous lines of therapy.

The study begins with a short dose-optimization stage evaluating two target dose levels. Cullinan plans to select a recommended Phase 2 dose and transition into a single-arm expansion cohort without starting a separate study. An exploratory cohort will also evaluate CLN-049 in patients with newly diagnosed AML carrying TP53 mutations.

The design follows an End-of-Phase 1 meeting with the U.S. Food and Drug Administration. Describing the study as potentially registrational does not mean the FDA has agreed to approve CLN-049 or guaranteed that a single-arm data set will support an application.

CLN-049 is an FLT3xCD3 T-cell engager designed to connect a patient’s T cells with leukemia cells expressing FLT3. Unlike treatments directed only at mutated FLT3, the candidate binds both mutated and non-mutated forms of the target, potentially allowing Cullinan to study it across a broader AML population.

“We believe CLN-049 has the potential to offer a novel immunotherapeutic approach for patients with AML,” Chief Medical Officer Jeffrey Jones said.

CLN-049 has received FDA Fast Track and Orphan Drug designations for relapsed or refractory AML. These designations may provide development incentives and additional regulatory interaction, but they do not establish the treatment’s safety or effectiveness.

Cullinan is also initiating a separate Phase 1/2 trial combining CLN-049 with azacitidine and venetoclax in newly diagnosed AML. The company expects to release an update from the ongoing Phase 1 dose-escalation study in December.

Financially, Cullinan reported approximately US$356 million in cash, cash equivalents, investments and interest receivable as of June 30, 2026. Management expects that capital to fund its current operating plan into 2029.

The company recorded a second-quarter net loss of US$53.7 million and spent US$44.4 million on research and development. Operating activities used approximately US$84.3 million during the first six months of 2026. Cullinan has no approved products and has not generated product-sales revenue, leaving its valuation dependent on clinical and regulatory progress across its pipeline.

Upcoming Catalysts

Sources used include Cullinan’s October 8, 2026 first-patient announcement, its August 6, 2026 financial and corporate update, its Form 10-Q for the quarter ended June 30, 2026 and the ClinicalTrials.gov record for the Phase 1 CLN-049 study. Information concerning CLN-049’s mechanism, therapeutic potential, study design and expected milestones originated primarily from Cullinan and has not been independently validated by Biotech Stocks Daily.

CLN-049 remains investigational and has not been approved by the FDA or another regulatory authority. The term “potentially registrational” does not guarantee that the Phase 2 study will produce sufficient evidence for a regulatory submission or approval. Fast Track and Orphan Drug designations do not establish safety, effectiveness or a favorable review outcome.

The Phase 2 study may encounter enrollment, safety, manufacturing, regulatory or operational delays. Its single-arm design may make interpretation dependent on comparisons with historical results. Early clinical findings may not be reproduced in larger studies, and the program may require additional controlled trials before any potential approval.

Financial information is reported in U.S. dollars and reflects Cullinan’s position as of June 30, 2026. The company has no approved products, has not generated product-sales revenue and expects continued research and development expenses and operating losses. Its projected cash runway into 2029 is a forward-looking management estimate that may change as clinical programs progress.
