Cognition Therapeutics Inc. (Nasdaq: CGTX) announced on October 22, 2026, that it has reached agreement with the U.S. Food and Drug Administration on key elements of a planned pivotal Phase 3 trial of zervimesine in dementia with Lewy bodies.

The discussions addressed patient selection, dosage, treatment duration and clinical endpoints. Cognition plans to enroll patients with mild-to-moderate dementia with Lewy bodies and randomly assign them to receive either 100 milligrams of oral zervimesine once daily or a placebo for nine months.

The study’s primary endpoint will use the Neuropsychiatric Inventory-2 to measure hallucinations and delusions. A key secondary endpoint will assess patients’ ability to perform daily activities using the Alzheimer’s Disease Cooperative Study–Activities of Daily Living scale.

“With FDA’s alignment on trial design, we are completing a protocol to support the planned registrational study,” President and CEO Lisa Ricciardi said.

Cognition has not announced that the trial has started enrolling patients. FDA alignment on the design also does not represent approval of zervimesine or guarantee that a future application will be successful.

Dementia with Lewy bodies is a progressive neurodegenerative disease that can produce cognitive impairment, hallucinations, delusions, movement difficulties and fluctuations in attention. There are currently no FDA-approved treatments specifically intended to slow the underlying progression of the disease.

Zervimesine, previously known as CT1812, is an experimental small-molecule drug that targets the sigma-2 receptor. Cognition is studying whether the treatment can prevent toxic protein oligomers from binding to neurons and damaging connections between brain cells.

The planned Phase 3 trial follows the exploratory Phase 2 SHIMMER study, which enrolled 130 patients with mild-to-moderate dementia with Lewy bodies. Participants received 100 milligrams or 300 milligrams of zervimesine, or a placebo, for 26 weeks.

SHIMMER met its primary objective of evaluating safety and tolerability. Cognition also reported favorable trends across exploratory measures of behavior, cognition, movement and daily function. However, the study was not sized as a confirmatory efficacy trial, and its clinical-effect findings require validation in a larger pivotal study.

Of the 130 randomized participants, 109 completed SHIMMER. Discontinuations associated with adverse events occurred in 4.5% of patients receiving 100 milligrams, 16.3% of those receiving 300 milligrams and 4.8% of the placebo group. The Phase 3 design uses the lower 100-milligram dose.

Cognition reported $34.8 million in cash, cash equivalents and restricted cash as of June 30, 2026, along with $21.6 million in remaining obligated grant funding. Its second-quarter net loss was $3.9 million. Management expects its resources to support operations into the fourth quarter of 2027, based on its existing business plan.

The company raised approximately $6.9 million in gross proceeds by selling 5.5 million shares through its at-the-market program during the first half of 2026. Approximately $68.1 million remained available under that program as of June 30, creating the potential for additional shareholder dilution if Cognition sells more shares.

Upcoming Catalysts

  • Completion and disclosure of the final Phase 3 protocol
  • Manufacturing of the newly patented form of zervimesine
  • Regulatory and operational updates ahead of trial initiation
  • Enrollment of the first Phase 3 participant
  • Additional financing, grant funding or partnership activity
  • Results from the separate Phase 2 START study in early Alzheimer’s disease

Sources used include Cognition’s October 22, 2026 FDA-meeting announcement, its October 13 patent announcement, its second-quarter 2026 financial results, its Form 10-Q, the SHIMMER record on ClinicalTrials.gov and the peer-reviewed SHIMMER paper published in Alzheimer’s & Dementia. Statements concerning zervimesine’s mechanism and development potential originated partly from Cognition and have not been independently validated by Biotech Stocks Daily.

Zervimesine remains investigational and has not received FDA approval for dementia with Lewy bodies or any other indication. Agreement with the FDA on elements of a trial design does not guarantee trial initiation, successful results or regulatory approval. The Phase 3 study has not started enrolling patients.

SHIMMER was an exploratory Phase 2 study with safety and tolerability as its primary objective. Its efficacy measures were exploratory, and favorable trends observed in a relatively small study may not be reproduced in a larger pivotal trial. Treatment discontinuations were higher in the 300-milligram group than in the 100-milligram and placebo groups.

Financial information reflects Cognition’s position as of June 30, 2026. The company has incurred recurring losses and expects additional development expenses. Its projected cash runway into the fourth quarter of 2027 is a forward-looking estimate. Use of its at-the-market equity program or other financing could dilute existing shareholders.
