Vanda Pharmaceuticals Inc. (Nasdaq: VNDA) announced that its Phase 3 study of VQW-765 in adults with social anxiety disorder failed to meet its primary endpoint. The company has placed the development program on hold while it analyzes the complete dataset.
The randomized, double-blind and placebo-controlled Study 3201 enrolled 546 adults between the ages of 18 and 65 at approximately 30 clinical sites in the United States. Participants received a single oral dose of VQW-765 or a placebo before undergoing the Trier Social Stress Test, which required them to prepare and deliver an interview-style presentation before an unresponsive panel.
The study’s primary endpoint measured participants’ self-reported distress during the presentation phase using the Subjective Units of Distress Scale.
Among female participants, the mean distress score was 65.6 for the VQW-765 group and 66.0 for the placebo group. The difference was not statistically significant, with a p-value of 0.8899.
The trial also failed its key secondary endpoint in the full population of male and female participants. The mean score was 64.7 with VQW-765 and 63.8 with placebo, producing a p-value of 0.6605.
Study 3201 was designed to confirm an efficacy signal observed in an earlier Phase 2 trial involving people with performance anxiety. The new results did not reproduce those findings.
“We are disappointed that Study 3201 did not confirm the earlier Phase II findings,” President and CEO Mihael H. Polymeropoulos said.
Vanda reported that VQW-765 was safe and well tolerated, consistent with earlier studies. The company did not provide detailed adverse-event results in its topline announcement.
VQW-765, previously known as AQW-051, is an oral small-molecule partial agonist of the alpha-7 nicotinic acetylcholine receptor. Vanda licensed the compound from Novartis and has been evaluating it as an on-demand treatment for acute anxiety associated with social situations.
The failed trial substantially weakens the clinical rationale for advancing VQW-765 in social anxiety disorder. However, the setback is limited to one development program. Vanda remains a commercial-stage pharmaceutical company with approved products, including Fanapt, Hetlioz, Ponvory and Nereus.
Financial capacity remains relevant as the company evaluates its pipeline. Vanda reported $50.5 million in second-quarter 2026 net product sales and a quarterly net loss of $62.5 million. Cash, cash equivalents and marketable securities totaled $170 million as of June 30, down from $263.8 million at the end of 2025.
